Mission Statement

The mission of this Foundation, a broad-based, non-profit community organization, which is exclusively educational and charitable, is to secure resources from individuals, corporations, community organizations and other foundations to be distributed in support of programs that benefit students in the Centennial School District, which will lead to the overall improvement in the quality of education and an enhancement of community support for public education.

The Foundation will fund projects that are not funded by the District’s General Operating Budget. Examples of such projects may include:

¨ Multi-Cultural Programs
¨ Environmental Projects
¨ Science Fairs
¨ Visiting Scholars and Artists
¨ School-To-Work Projects
¨ Field Trips
¨ Other Ideas

The Foundation will not fund salaries, general school equipment costs, general school supply costs, transportation within the district, and food for events.

Who can apply?

Any Centennial School District teacher can apply for funds with Principal’s approval of project.

Who reviews and recommends projects to be funded?

The Centennial Education Foundation Board of Directors is made up of volunteers from the community, businesses, and the District. The board assigns members to a grant review committee. This committee reviews and scores each grant using a standard scoring rubric. Grant Reviewers evaluate projects based on:

-- Impact on the curriculum
-- Affect on the students
-- Quality of planning
-- Number of students participating
-- Financial soundness

When is the application due?


All applications are due in the Foundation Office at the Johnsville Administration Building: Attention: Donna Mooney, by 4:00 p.m. on Wednesday, February 15, 2006. Any application received after the deadline will be disregarded. All applications must be type written in 12 font. All applications sent electronically must be sent through the building principal to ensure principal’s approval.

Please be sure to complete the “Cover Page”. The Summary/Overview on the cover page should be one/two paragraphs in length and should cover the main points that are outlined in the application.

What are the details of funds awarded?


There is no minimum amount of grant funds awarded, but the maximum amount awarded for any one grant is $500.00. In addition, if the full amount of the grant awarded is not utilized, the balance of the funds must be returned to the Foundation with the required reconciliation sheet. Please indicate on the Program Budget Summary where the balance of the funds will come from if the project exceeds the maximum $500.00 grant.

Is there a deadline to use the awarded funds?


Yes. All awarded funds must be totally expended by June 2007. In the event of an extenuating circumstance where the recipient is unable to conduct the project and expend the awarded funds by the set deadline; application in the form of a written explanation why project is delayed must be forwarded to the Board for review. If awarded funds are not expended under normal circumstances, Board has the right to suspend funds and applicant must reapply in the next application year.

Listed below you will find questions that are included in the narrative section of the application. Included with the questions are some key points that will help you with your responses and will be used in the evaluation process.

Please avoid referring to any Centennial School District staff member(s) or school(s) by name when completing the application summary and narrative section of the application. This is so that our Grant Reviewers can concentrate on the application in an unbiased way.

1. General Program Description

Please describe the proposed project: Why do you want to do this project? What are the overall benefits of this project to Centennial students? How does this project relate to the mission of the Centennial Education Foundation? This is your opportunity to present your program and to convince the foundation of the importance and desirability of the program.

2. Project Detail

Please use the following questions as a guide when you write this narrative:
-- What are the goals of this project?
-- Who does this project benefit/target? What grade level/subject area?
-- How does this project relate and expand on the curriculum?
-- What activities will the students be involved in as a result of this project?
-- Who will be responsible for the project implementation? (If this is a group project among teachers then define roles-do not use names)
-- What is the number of students impacted? (Maximum number of points awarded to student groups of 25 or more)
-- Who is your guest speaker and did you attach their resume?

3. Project Budget

Included in the application is a “Project Budget Summary” sample. Along with the budget summary complete a budget narrative by describing expenditures and how they relate to the project activities. Describe any matching funds that may support the project. (This is a critical part of the application. The budget must match the activities that have been described in question 2)
